Ectopic ACTH syndrome, also known as ectopic adrenocorticotropic hormone syndrome, is a rare medical condition characterized by the production of adrenocorticotropic hormone (ACTH) from a location other than the pituitary gland. Normally, the pituitary gland in the brain secretes ACTH, which stimulates the adrenal glands to produce cortisol, a hormone essential for regulating various bodily functions. In cases of ectopic ACTH syndrome, a tumor or abnormal growth outside the pituitary gland releases ACTH, resulting in excessive cortisol production.
The most common cause of ectopic ACTH syndrome is a tumor, typically of neuroendocrine origin, located in areas such as the lungs, pancreas, thyroid, or thymus gland. These tumors produce ACTH independently, leading to increased cortisol levels. Symptoms of this syndrome may include weight gain, high blood pressure, muscle weakness, easy bruising, glucose intolerance, osteoporosis, and mood changes.
Diagnosing ectopic ACTH syndrome involves measuring cortisol levels, ACTH levels, and performing imaging tests to locate the tumor. Treatment options depend on the size, location, and aggressiveness of the tumor, as well as the patient's overall health status. Surgical removal of the tumor is often the preferred approach whenever possible. Additionally, medications such as ketoconazole, mifepristone, or etomidate may be used to manage the symptoms associated with excess cortisol production.
